How to Use

1. Visit www.locateprovider.com or call the toll free number listed on your membership card.
   
   
2. Please provide the customer satisfaction specialist with your name, membership number, and zip code. You will be provided with the names and telephone numbers of the participating physicians within your community.
   
   Should you currently be working with a chiropractor with whom you are happy, you may provide the customer satisfaction specialist with their name, address, and telephone number. We will be happy to invite the chiropractor to apply for participation in the network. Unfortunately, UHS cannot guarantee acceptance by the program administrator or the chiropractor.
